We are a licensed Medicaid provider for the State of Georgia providing assistance for low- to middle-income individuals who have physical or mental limitations making independent living inadvisable or unsafe.
How We Help Georgia Seniors and Families
Working closely with the medical community, we implement Georgia Medicaid’s SOURCE and CCSP programs to assist in placing qualifying seniors into licensed personal care homes in Atlanta. Once residents are placed, our professional nursing staff coordinates the care and case management component of the programs to supervise the care received by the seniors in the homes to ensure their health and well-being.
Our Home Care Services
Unlike nursing homes and large assisted-living facilities, licensed Atlanta personal care homes are single-family homes located in lovely neighborhoods throughout Atlanta. These family model homes for one to six residents provide single or shared bedrooms with full access to the rest of the house. Residents are served three nutritious meals plus snacks every day and are provided with assistance as needed for such tasks as bathing and dressing. Laundry, housekeeping, and transportation services are also provided.
