Providing Affordable Senior Housing in Atlanta
There are two Medicaid programs in Georgia that help qualifying seniors get affordable senior housing in Atlanta and pay for assisted living services:
- SOURCE (Service Options Using Resources in a Community Environment) is available to those who are receiving or are eligible for Medicaid/SSI (Supplemental Security Income) who meet the Intermediate Nursing Home level of care.
- CCSP (Community Care Service Program) is for those seniors who have limitations making it inadvisable to live independently, are eligible for Medicaid, and have an income of $400 to $1809 per month.
Care Coordination and Management
Personal Care Homes Atlanta provides on-going care coordination and case management services to SOURCE and CCSP clients. After evaluating the level of care needed and facilitating the client’s placement into an appropriate personal care home, our professional staff continues to monitor and coordinate the client’s care and health with visits from a registered nurse every two weeks.
Each Is a Quality Personal Care Home in Atlanta
Personal Care Homes Atlanta contracts with Family Model Licensed Personal Care Homes. These privately owned homes provide housing within the community for older physically or mentally disabled clients.
Each home provides 24/7 nonmedical care for one to six residents in a comfortable residential setting that provides full and easy access to all areas of the house. Meals and snacks are provided along with services such as laundry, transportation, personal care, and more as needed. The homes are inspected yearly to ensure the safety of the residents and the compliance of the home with Georgia state regulations.
